Health Care Provider Referrals

Thank you for your interest in the UCSD Shiley-Marcos Alzheimer’s Disease Research Center. Our ADRC provides you and your patients with a wide array of resources.  We do not replace you as health care provider, as we are not a treatment center.  Rather, we seek a cooperative relationship in which our role is to give patients feedback on their current status and an opportunity to make a valuable contribution to research investigating Alzheimer’s disease. 

What the Shiley-Marcos ADRC Offers Your Patients:

  • Clinical Drug Trials: potential to benefit from promising new treatments
  • Social Services: support groups for patients and caregivers as well as organized activities and social work consultation
  • No Charge: there is no charge for studies and some imaging trials will compensate participants

What the Shiley-Marcos ADRC Offers You:

  • Quarterly updates on your patients’ enrollment status
  • Results and findings of testing, with your patient’s permission

 

We realize that you are extremely busy and we want to make the referral process as simple as possible.  Click the provided link to access the Secure Physician Referral Forms.  These can either be completed and submitted on our secure online form, or they can be printed and either faxed or mailed to our La Jolla office when a patient indicates interest in learning more about Alzheimer’s disease or participating in research.   We are committed to being prompt and professional when contacting your patients.  We will be more than happy to give you updates regarding the patients you refer to the ADRC as long as the patient provides authorization. 

Please register on our site using the secure online registry and request form for physicians.  Registering will automatically place you on our mailing list, so we can send you updated information about our currently enrolling trials, as well as the most recent issue of our newsletter, Currents.  We would also be happy to mail you a “physician packet”, which includes hard copies of the referral forms found online, an informational flyer that may be placed in your waiting room for patients to read, and a small pad with local contacts and resources.  Please feel free to contact us at (858) 622-5800 if you would like more information or have questions.
